2018 Live Well San Diego Public Health Champion Awards Call for Nominations The County of San Diego Health and Human Services Agency (HHSA), Department of Public Health Services is proud to announce the Live Well San Diego 17th Annual Public Health Champion Awards. These awards recognize the extraordinary achievements, by individuals and organizations, in areas of public health services that protect and promote the health of San Diego County residents. The awards were created to recognize the role of public/private collaboration in advancing optimal health and wellness in the County’s diverse populations. This year’s theme award will recognize achievements related to National Public Health Week theme: "Healthiest Nation 2030: Changing Our Future Together." Public Health Champion Awards is a program of Live Well San Diego, a long-term vision for improving the health and wellness of all San Diegans. The Awards Program will be held at the County Administration Center on April 6, 2018 from 10 am to 12 pm in the Board Chambers Room 310, in conjunction with National Public Health Week. This year the categories are as follows: 1. Public Health Champion Award – Theme: "Healthiest Nation 2030: Changing Our Future Together." 2. Public Health Champion Award – Individual 3. Public Health Champion Award – Organization 4.
